- Patient’s portion of bill due on day of service

- There will be a $50 reinstatement fee if your program lapses
- No refunds will be issued at any time if participant decides not to utilize dental plan

Free Whitening
At Midlakes Dental all new patients are encouraged to take advantage of our free lifetime whitening. We will provide you with a fitted mouth guard as well as the whitening gel at every 6-month visit in order to help achieve the bright, white smile you’ve always wanted while keeping to efficient and affordable dental care.
